A. Classifications of scheduling:
            1. Firm – commitment for services according to terms of this agreement.
2. Tentative – expression of interest from prospective client with first right of refusal, but no firm commitment by either party.
 
B. Time frames for scheduling:
            1. Firm requests may be made any time scheduled dates are available, but may carry a cancellation fee. (see Cancellations below)
            2. Once a firm commitment is made, you will receive a written confirmation with the conditions of our agreement for your signature and return.
            3. Tentative requests may only be made within 60 days of current date and carry no cancellation fee.
 
C. Cancellations:
1. Firm commitments cancelled at more than 2 weeks out may be billed at 20% of the minimum fees for the length of the committment.
2. Firm commitments cancelled 1 to 2 weeks out may be billed at 50% of the minimum fees for the length of the committment.
3. Firm commitments cancelled lees than 1 week out may be billed at 100% of the minimum fees for the length of the committment.
4. Tentative requests may be cancelled at any time without penalty or converted to “firm”.
 
D. Minimum billable hours:
1. The total minimum daily time “block” shall not be less than 4 hours.
2. The daily scheduled time “block” shall start with the first requested work for that day, and conclude with the last agreed work for that day.
3. The daily scheduled time “block” shall allow for up to 1 hour lunch between the hours of 11:30AM and 1:30PM.
4. Unscheduled time (not required for documentation, pt. care or meetings)  between the start and conclusion of requested work in excess of the minimum daily time “block” shall be billed at 50% of the fixed hourly rate.
5. I will provide you with an invoice (due upon receipt) at the end of the assignment or every two weeks, whichever comes first.
 
E. Mileage – Mileage in excess of 30 miles one way will be billed at the going IRS rate.
 
F. Emergency scheduling:
1. Give me a call if you have an emergency staffing situation.  I may have had recent changes to my schedule not reflected on the online calendar.
 
H. Cooperation and flexibility:
It is the intent of this agreement to serve the best interests of all parties. Continual communication and advance notification of changes is essential in making this program work for everyone.
